As the world gets fatter, the ultimate low-carb, high-fat eating regimen emerges as the fashionable diet trend of the moment. But is it right for you? Here’s the lowdown from experts

With new fad diets popping up every now and then, you are bound to wonder what they are all about. The “it” diet of the moment is the ketogenic diet. Celebrities can’t get enough of it, stars swear by it. Food and beverage manufacturers are launching new keto snacks every day. The global ketogenic diet industry estimated at USD 10.3 billion is rising at a fast clip of 8 percent a year.

This diet was created way back in the 1920s as a treatment for epilepsy. It was widely used until anti-epileptic drugs came on the market in 1938. It started gaining attention once again from the 1990s. Not so much for epilepsy, as for providing metabolic protection against the ailments of modern times: obesity, diabetes, heart disease and so on.

DIGGING INTO RESEARCH

Here, we dig into a recent article by Shilpa Joshi and Dr Viswanathan Mohan¹. Joshi is a dietician with the Mumbai Diet and Health Centre. Dr Mohan is the Chairman of Dr Mohan’s Diabetes Specialities Centre & President of Madras Diabetes Research Foundation in Chennai.

Published in the Indian Journal of Medical Research, September 2018, their research—“Ketogenic diets: Boon or bane?”—analyses threadbare the pros, cons and controversies around the diet. Although meant for the academic community, here we present the easy-to-understand facts, to make it accessible to all.

WHAT’S THE K DIET?

The ketogenic diet is, typically, an eating pattern marked by very low carbohydrates and very high protein and fat. In fact, extremely high percentage of fat is often used in this diet plan. The classic keto diet requires about 90 per cent calories from fat, 6 per cent from proteins and 4 per cent from carbohydrates. And there are no restrictions on the type of fat to be consumed.

The purpose of this diet is to drastically reduce carbohydrate consumption (below 50 g/day) and shift the body’s primary fuel source from carbs to fat. As carbs are cut out, the glucose reserves of the body become insufficient. And the body starts to mimic the effects of fasting. An alternative source of energy emerges: the ketone bodies, produced in the liver by the breakdown of fats.

WHAT’S ON OFFER

One of the unique aspects of the keto diet is its flexibility. Although the basic principles are universal, there are several ways to do this diet. Explore the different types of keto diets:

— The Standard Ketogenic Diet (SKD) is very low-carbohydrate (about 10 per cent), with moderate-protein (20 per cent) and high-fat diet (about 70 per cent).

— Cyclical Ketogenic Diet (CKD) involves periods of higher-carbohydrates in between the Keto diet cycles. For instance, five Ketogenic days, followed by two high-carbohydrate days.

— Targeted Ketogenic Diet (TKD) permits the addition of more carbohydrates, to coincide with intensive physical workout regimes.

— High-protein Ketogenic Diet (HPKD) allows 35 per cent protein, 60 per cent fat and five per cent carbohydrates.

The SKD is the most researched and recommended. HPKD has also been tried out extensively. CKD and TKD are recent additions, used mostly by bodybuilders or athletes. The SKD is the most researched and recommended, and the rest of this article will deal with SKD.

FAT OF THE MATTER

To Joshi and Mohan, the context of the keto diet is the rising wave of obesity in the last few decades, globally and in India. Obesity puts people at risk of type 2 diabetes, high blood pressure, heart disease, bone and sleep disorders, some forms of cancer and even depression. Yet, it is hard to manage obesity medically. “This leads to frustration among individuals with obesity,” write Joshi and Mohan, leading many to resort to extreme dietary interventions. Latest research shows that people who follow the diet lose more weight than those who follow balanced diets.

The keto diets seem to benefit in blood sugar control, reduce total cholesterol, serum triglycerides and increase high-density lipoprotein cholesterol. There are reported benefits on cancer and neurological disorders—Alzheimer’s disease to epilepsy. There are adverse effects, too: muscle cramps to bad breath, changes in bowel habits, keto-flu and loss of energy. “Monitoring individuals on keto-diet closely, once or twice a month, for blood glucose, ketones, cardiac and other parameters is essential,” point out Joshi and Mohan.

THE CARB QUESTION

Indian health and diet have complicated relationship with carbohydrates. Study after study has shown in the last few years, that Indians consume very high percentage of calories in their diets from carbohydrates.

— In 2009, The Chennai Urban Rural Epidemiology Study (CURES)² conducted by Dr Mohan and others on people with newly-detected type 2 diabetes had revealed how carbs constituted the major source of calories in South India.

— It was followed by the STARCH study³ of 2014. Conducted by endocrinologist Dr S.R. Joshi, this dietary survey on people with type 2 diabetes showed how Indians got 65 per cent of their dietary calories from carbs³.

— In 2017, came the PURE Study4, conducted across 18 countries from five continents. Dr Mohan, a collaborator of the study that probed the linkages between fat-carbohydrate intake and heart disease-death. It was found that high carb intake (more than about 60% of calories) was associated with high death toll. In contrast, higher fat intake was found to have lower risks.

— A 2018 study5 now shows a long-term adverse effect on life-expectancy with both low-carb and high-carb diets. Led by Dr S.B. Seidelmann of Brigham and Women’s Hospital, Boston, US, the study suggests extremely high-carb diet (60% and above) as well as very low-carb (<30% carbs) diets increases the risk of dying. In fact, the risk was lowest when the carb intake was between 50 and 55 per cent; plant-based proteins and fats were also found to be more protective than animal-based proteins and fats.

— And finally the latest study in 20206 from the PURE study with Bhavadharini Balaji as the lead author and Dr.V. Mohan as the Second author has conclusively shows an association between excess white rice intake and new-onset type 2 diabetes.

TOWARDS SOLUTION

Should you go for the keto diet? Joshi and Mohan suggest “moderation.” The diet shows a dramatic improvement in the short-term. But is it sustainable? The researchers explain that initially people are thrilled with the weight-loss and the excellent diabetes control they get from keto diets. Gradually, however, they get bored, many feel weak and slowly increase carbohydrate intake. “Soon they are back to their original weight and diabetes control,” they add.

What is the long-term impact of not having all food groups on the plate? Low-carb diets lead to lower intake of vegetables, fruits, grains and greater intake of fat—a detrimental slope in the long term because it is likely to stimulate inflammatory pathways, oxidative stress and biological ageing. There are issues of serious complications like sensitivity of the liver to insulin, micronutrient deficiency. As “the pendulum of nutrients swing on either side, one must be vigilant of the balance and interplay of nutrients,” they warn.

Here are some recommendations from them, if you plan to follow the keto diet:

Keep carbohydrates consumption to 50-55 per cent of your diet

Use complex carbs and whole grains (brown rice or whole wheat)

Take about 20-25 per cent protein (preferably vegetable proteins, such as legumes and pulses)

The remaining 25-30 per cent from healthy fats like monounsaturated fats (groundnut oil or mustard oil), nuts and seeds

Take plenty of green leafy vegetables. This will make your diet sustainable for long, make it less risky, more healthy and also help prevent non-communicable diseases such as diabetes, cardiovascular disease and certain cancers.

So, pause, take care before you embrace any extreme diet. Many more studies are needed before keto diets can be accepted widely.

It’s frustrating to be a Diabetes Doctor in India. The disease has reached epidemic proportions, yet far too many people are still clueless about its signs and symptoms, how vicious it can be or how early diagnosis can save a life and a limb. As doctors continue to treat well-developed complications in patients coming for the first time, the need to spread the word about the signs and symptoms of diabetes becomes urgent.

The problem is, diabetes can stay hidden for long, sometimes for years. And the longer it stays hidden the more it increases hospitalization from heart disease, stroke, kidney failure, vision loss and amputation. Now, it seems, fatal outcomes from Covid-19 are afflicting people with diabetes much more.

So let us turn the spotlight to that key question: “Do you have diabetes?” It is said, knowledge is power. So long you are armed with information and awareness, you can control, manage and defeat diabetes. And always consult a doctor to know in detail about diabetes, its symptoms, complications and the treatment options that are best for you.

TRUTH BE TOLD

The truth is: diabetes is a disease in which the body cannot control sugar levels in the blood. People with diabetes have a problem with insulin—the hormone which keeps sugar levels safe, between too high (hyperglycaemia) and too low (hypoglycaemia). When diabetes is uncontrolled, glucose and fats remain in blood and, over time, damage vital organs. In the end, the complications may be seriously disabling or even life-threatening.

Some people need to be extra careful: older age, family history, overweight (especially, around the belly), unhealthy eaters, couch-potatoe lifestyle. And, also, those who have had diabetes during pregnancy.

THE SIGNS AND SYMPTOMS OF DIABETES

High blood sugar

Or hyperglycaemia, includes the following major symptoms:

-Excessive thirst

-Frequent urination

-Hunger

-Weight loss

-Blurred vision

-Itching

-Skin infections

Low blood sugar

Or hypoglycaemia, shows the following (and more):

-Extreme hunger

-Feeling nervous or jittery

-Cold, clammy, wet skin and/or excessive sweating

-Rapid heartbeat

-Numbness or tingling of the fingertips or lips

-Trembling

-Giddiness

Constant fatigue

Do you feel tired all the time, lack energy, find it difficult to carry out simple everyday tasks, or feel low and depressed? If you do, take care. In diabetes, high blood sugar levels affect the body’s ability to get glucose from the blood into cells to meet our energy needs. The tiredness is often brushed off as “too much work,” but if the fatigue continues even after one has slept enough, it is time to wake up and consult the doctor.

Excessive hunger

Do you feel hungry all the time? Or do you feel hungry even after you eat? That’s because, while your body is not converting the food you eat into energy, muscles and other tissues are sending a “hunger” message, to get more energy into the system. Sometimes, the hunger pangs become so intense that one ends up binge eating—snacking on starchy, comfort foods. That can relieve hunger for a while, but it comes back again the moment sugar levels dip.

Excessive thirst & urine

Most people with diabetes feel thirsty constantly and tend to drink more than people with normal blood sugar levels. If you are constantly thirsty (regardless of the weather or physical activity), you may have the condition. Thirst is linked to another common diabetes symptom: urinating more than normal. High sugar in blood leads to high glucose in the urine, resulting in increased urination. Be alert, if you are waking up more than once at night to go to the washroom.

Unexplained weight loss

Everybody gains or loses some weight now and then. But if you haven’t gone on a diet or ramped up your exercise routine and your weight is still dropping—relatively quickly—it is time to talk to your doctor. In fact, a red flag to look out for is the answer to a question: “Are you snacking more than normal yet losing weight?” In diabetes, as your body doesn’t use insulin effectively, and the glucose doesn’t arrive in your cells, your body thinks it’s starving and starts creating energy by burning fat and muscle. This causes unexplained weight loss.

Vision problems

A typical complaint from people with diabetes is that their vision is not improving despite the use of glasses. That’s because, high blood sugar can set off a range of eye problems: spots or dark strings floating in your vision; flashes of light; ‘holes’ and blurry vision; cataracts (the lens gets cloudy); glaucoma (pressure builds up inside your eye); retinopathy (damage to small blood vessels in your retina). Early signs can be treated, although cataracts often need surgery. Remember, diabetes is the primary cause of blindness in adults.

Mental problems

Changes in blood sugar levels can have negative effects on the brain. Depression, mood swings, restlessness, lack of concentration, difficulty thinking clearly, feeling shaky, sweaty, anxious, sad (especially in the mornings), low and irritable, no longer finding pleasure in activities once enjoyed, experiencing insomnia or sleeping too much, loss of appetite or binge eating—all these are now being linked to diabetes, both as symptom as well as consequence.

Slow wound healing

If you notice that your cuts, scratches, grazes and blisters take several weeks to heal, it may just be an important sign that you have diabetes. High blood sugar complicates the immune response in the body. As a result wounds do not repair efficiently, resulting in slow healing.

Tingling and numbness

Pain, numbness, burning sensation or tingling in hands and feet, are often things that people with diabetes complain of. That’s because, high blood sugar levels can cause poor blood circulation and nerve damage in hands and feet, resulting in conditions that are collectively known as neuropathy. They can be vital symptoms, too.

Skin changes

There are warning signs of diabetes that can appear on your skin: from yellow-red-brown patches on the skin to dark, velvety bands in the folds of the skin (especially in the neck, armpit, groin, or elsewhere); hard, thickening skin on the fingers and toes; large blisters that suddenly arise on the hands, feet, legs, or forearms (resemble serious burn blisters); extremely dry, itchy skin; scaly patches around the eyes.

LAST BUT NOT LEAST !

Here comes the handgrip test. It’s not a symptom, but a sign. You can foretell if you are (or will) move towards diabetes. Answer the following questions: do you find it difficult to open a bottle, brush your teeth or hold a steering wheel? A weak handgrip is usually linked to normal ageing, decrease in muscle mass, diseases like arthritis and nerve damage in the hands and wrists. But now (published in Annals of Medicine, September 2020) scientists are measuring the muscular handgrip strength of men and women without a history of diabetes, to detect their risk of developing the disease.

So test the strength of your handgrip and consult your doctor, if any of the above mentioned symptoms resonate with you.

Brown Rice vs White Rice: Which is better for you?

The effect of wholegrain rice on blood glucose is a new and emerging field of research. The scientific studies have largely been conducted on Western populations. Now the pioneering work of a handful of researchers is bringing new hope to the rising burden of diabetes in India. Here is one such study.

When a nation suffers from an overwhelming disease burden, it is natural to examine the dominant food people consume. The World Health Organisation (WHO) acknowledges the crucial role of diet and nutrition in the prevention of chronic diseases.

India is often called the Diabetes capital of the world. And scientists are probing the link between the disease and the way we eat now. Staple foods are under the scanner.

Dr Viswanathan Mohan, along with his collaborators, has been studying the role of the common cereal staple, white rice, behind the rising diabetes prevalence.

Here is a review of his research on improving the Indian diet by replacing white rice with the nutritious whole grain, brown rice. The research was conducted by the scientists of Madras Diabetes Research Foundation, Chennai, India, the Harvard School of Public Health, Boston, US and the Brenner Centre for Molecular Medicine, Singapore.

The study was published in the peer-reviewed journal, Diabetes Technology & Therapeutics in 2014, as “Effect of Brown Rice, White Rice, and Brown Rice with Legumes on Blood Glucose and Insulin Responses in Overweight Asian Indians: A Randomized Controlled Trial”. This is a simplified version of the scientific study, to make it user-friendly for people with diabetes and other interested readers.

A STAPLE FOR LONG

Staple foods are foods that form the dominant part of the diet of a community or country. Typically, they are foods that depend on and develop from the soil and climatic conditions of a region. Rice feeds almost half of humanity, reports the Food and Agriculture Organization of the United Nations (FAO). Asia is the world’s largest rice-producing and rice-consuming region. India is the second-largest producer of rice in the world, next to China.

Archaeological evidence suggests that rice has been the basis of Indian diet for long, even as far back as 6500 BC. India has had some 110,000 varieties of rice—brown, red, golden, even black—typically, processed by labour-intensive, hand pounding. India’s diversity of rice has been one of the highest in the world—from aromatic long-grains to round, medium-grains, from glutinous to parboiled rice (processed differently after harvesting). In the last 50 years, we have lost tens of thousands of rice varieties in India, that are nutritious and healthy.

THE WHOLE GRAIN

The rice grain has three parts. The hard outer layer is the fibre-rich bran, whose pigmentation determines the colour of the rice, the most common being the brown rice. The nutrient-rich core is the germ (the embryo) of the grain, that grows into a new plant. Bran and germ are rich in protein, fibre, iron, vitamin B and omega 3:6 fatty acids, in addition to carbohydrates. Endosperm, the tissue that surrounds the embryo, provides nutrition in the form of starch. Covering all this is the inedible protective outer shell, the husk or chaff or hull.

A whole grain contains the entire grain: bran, germ and endosperm (the husk has to be removed before rice can be cooked). It’s nutritious, packed with vitamins, minerals, antioxidants, essential amino acids and fibres. It has a low Glycemic Index (a measure of how fast the body converts carbs into sugars) and is better for people with type 2 diabetes.

It also contains compounds (lignans) that lower blood pressure, fat in the blood and the risk of heart disease. Brown rice has consistently been shown to aid weight loss and help maintain healthy body weight.

THE POLISHED GRAIN

Whole grain rice is highly nutritious but has a short shelf life. In order to increase the shelf life of rice, new milling technologies have displaced the healthy whole grain by removing both the bran and germ. The result is highly polished, starchy refined rice—or white rice—which is nutrient-depleted since endosperm is primarily carbohydrates with a little bit of protein. While brown rice has a short shelf life of about six months (unrefrigerated), white rice stays on the shelf for 4–5 years. The white rice is then processed to improve taste and enhance cooking properties. All this, at expense of nutrients.

Unlike unpolished brown rice, white rice lacks plant compounds called phytochemicals—such as polyphenols, oryzanol, phytosterols, tocotrienols, tocopherols, and carotenoids—as well as vitamins and minerals, that confer protection against cell damage, diabetes, heart disease and cancers. White rice is “empty calories,” since it loses its main sources of micronutrients, fibre, iron, and omega 3:6 fatty acids. With a higher GI, the carbs in white rice are turned into blood glucose more rapidly than those in brown rice. This is one reason why white rice has been associated with a higher risk of type 2 diabetes, obesity, blood pressure and heart disease.

THE NUTRITION TRANSITION

Today, our diets have undergone a rapid and fundamental change, called a “nutrition transition.” It means, an increased consumption of refined carbohydrates, unhealthy fats and foods of animal origin, associated with an increased risk of obesity and type 2 diabetes.

With rapid socio-economic change, abundant food availability (at least among the affluent classes), enhanced calorie consumption, a decline in the activities of daily living coupled with a dramatic increase in sedentary activities, easy access to high-calorie unhealthy food—all contribute to the problem. Studies on the health impact of refined carbohydrates and of whole cereals are of great importance in this context.

WHAT THE STUDY SAYS

About 15 overweight and/or obese Indian adults (age 25–45) without diabetes took part in the study. Identical test meals, except for the type of rice and the addition of legumes (50 g/day), were provided for five consecutive days.

This was the first study to assess glucose profiles using continuous glucose monitoring technology (Medtronic MiniMed iPro™2) to evaluate the effect of replacing refined grains with either a whole grain or with whole grains with legumes among obese Indians.

The average percentage difference in total blood glucose levels over five days was 19.8 percent lower in the brown rice group than in the white rice group. Brown rice with legumes further decreased the glycemic response (22.9 percent lower compared to white rice). The change in fasting insulin was lower for brown rice (57 percent) and brown rice with legume (54 percent), compared to the white rice group.

POINT TO REMEMBER

The glycemic index (GI) values of white rice vary and are influenced by several factors, including the degree of processing, the amylose (a type of starch in rice) content and cooking time. A key point of the study is that GI values of popular Indian white rice varieties are higher compared to whole grains, such as brown rice and with legumes.

Studies have assessed the beneficial effects of low GI diets on 24-hour glucose profiles, using continuous glucose monitoring. No such study has assessed the effects of replacing cereal staples like rice in general and in particular among overweight and obese adults people in India.

This has been the first such study.

A complex disease made even more complex by technical terms. Here’s how to get on top of all the medical buzzwords

Jargon is the specialized language used by insiders in a field. The world of medicine is famous for the use of unintelligible, confusing and hard-to-understand words. Jargon make a lot of sense to doctors, for easier communication while racing against the clock. For patients, however, the technical and scientific terms make it difficult to understand a disease and its management. These are also words that they often misunderstand or feel too embarrassed to ask and clarify.

Here is a quick guide to all the difficult and abstruse words related to diabetes.

Autoimmune disorder

When your immune system attacks and destroys healthy body tissues by mistake. There are certain types of diabetes that belong to this category.

Beta cells

Cells that make insulin, the hormone that controls the level of glucose (a type of sugar) in the blood. These cells are found within clusters of cells called islets of Langerhans in the pancreas.

Blood glucose level

The sugar content (glucose) of your blood at a given time, the most important measure in diabetes. If the level goes too high, there isn’t enough insulin in the blood. If it is too low, there is too much insulin. Doctors tell you the range of blood sugar level you should have, the target, and that is the basis of managing diabetes.

Bolus

Bolus secretion is the right amount of the hormone insulin released by the pancreas, after we eat, to process the carbohydrates in a meal. People with type 1 diabetes need to take a form of insulin that replicates the bolus secretion, called bolus insulin.

Carb Counting

Carbohydrate, as you know, is one of the three main energy sources for the body, the others being fat and protein. Your body breaks down carbohydrates to get glucose, which then provides energy to the body. Carb counting is the concept of managing food intake and blood sugar for people with diabetes. It involves counting the number of carb grams in a meal and matching that to your dose of insulin. Most useful for those who take insulin several times a day.

Coma

Usually refers to a clinical condition when a person is unconscious. This is a severe emergency that can be brought about by very high or low blood sugar levels or to an immediate cause example a brain stroke or an accident.

Continuous glucose monitoring

CGM is a sensor inserted just under the skin, usually on the belly or arm, that tracks blood sugar levels 24/7, at regular intervals, translating them into dynamic data. Can alert you if your sugar levels go too high or too low.

Glucose

Comes from the Greek word for sweet. It’s the basic sugar from food you eat, that your body uses for energy. As it circulates through the bloodstream to cells, it is called blood glucose or blood sugar.

Hba1c

A blood test used to measure the average blood sugar levels over the last three months. It helps your doctor see how well your blood glucose level has been controlled on an average.

Hyperglycaemia

A state of high blood sugar (high blood glucose).

HDL

High density cholesterol or Good cholesterol.

Hypoglycaemia

A potentially dangerous state of low blood sugar (low blood glucose).

Impaired glucose tolerance

It is also called prediabetes, when you have high blood sugar but not high enough to be diagnosed as type 2 diabetes. It means you are at higher risk of type 2 diabetes.

Insulin

The hormone that helps the body use glucose by keeping the levels of glucose in the blood under control. People with type 1 diabetes don’t have this hormone; people with type 2 diabetes either don’t have it or their bodies aren’t able to use it

Ketosis and Ketoacidosis

Both are linked to ketones, or fatty acid molecules that are made in the liver when your body doesn’t produce enough insulin to convert glucose into energy. The body starts breaking down fat for energy and ketones are produced as an alternative source of energy. The build-up of ketones can poison the body, leading to conditions like ketosis and ketoacidosis. More common with type 1 than type 2 diabetes. Ketosis happens with much lower levels of ketones, while ketoacidosis is a dangerous condition that happens with a higher build-up. Ketotic coma (diabetic coma) is an extremely dangerous state, which if untreated, can lead to death.

Endocrine system

The bodily system that produces hormones. Diabetes is an endocrine disorder, as insulin is a hormone.

Fasting blood glucose

It is one of the ways that diabetes is diagnosed. It measures the blood glucose level after fasting overnight.

Gestational diabetes

Diabetes during pregnancy, when women are found to have unusually high blood glucose levels. The symptoms usually disappear after delivery. Those who develop gestational diabetes, however, are more likely to develop type 2 diabetes later on.

Glucagon

A hormone that raises blood glucose level. Made by the pancreas, it counteracts the effects of the hormone insulin. If someone with diabetes has very low blood glucose, then a glucagon injection is sometimes delivered to help raise the blood glucose level.

LDL

Low density cholesterol or Bad cholesterol.

Macro and micro vascular complications

If blood glucose level is not managed well, over time it can lead to serious complications, including damage to major blood vessels (the macrovascular system) and tiny blood vessels (the microvascular system). Complications in the former lead to plaque build-up in the arteries, which can lead to a heart attack or stroke. Complications of tiny blood vessels due to diabetes can lead to problems with the eyes (retinopathy or cataracts), kidneys (nephropathy), and nerves (neuropathy).

Multiple Daily Injections

MDI, in brief, is the insulin regimen adopted by most people suffering from type 1 diabetes.

Nephropathy

Kidney damage due to chronic high blood sugar levels in diabetes.

Neuropathy

A term for all the nerve damage caused by chronic high blood sugar levels in diabetes. Can lead to amputations in severe cases.

Pen & Pump

Both are small, portable and self-contained devices that people with diabetes use to take insulin. Both can be used discreetly. Pens would be fitted with a pre-filled cartridge, a dial for measuring dosage and a disposable needle. The cartridge can be disposable or reusable.

Pumps are small computerised devices that mimic the way the human pancreas works. Connected 24/7, it delivers tiny doses of rapid-acting insulin around the clock through very thin plastic or metal tubes inserted just under the skin. You can press buttons to deliver extra doses at meal times.

PP

Means post prandial, or a period of time after eating, usually recorded as 2 hours

Rapid-acting insulin

A quick-acting dose of insulin, as part of your treatment regime, because it mimics natural Insulin spike.

Retinopathy

When the retina (back of the eye) is damaged due to high blood glucose and vascular disease, causing gradual loss of vision

Sugar substitutes

Or artificial sweeteners are often used by people with diabetes. They have fewer calories than foods made with sugar. Some recent studies, however, have found that the benefits of substituting sugar-sweetened foods and beverages with those that have been sweetened artificially may not be as helpful as once thought.

Quick-acting carbohydrate

Carbohydrate that raises blood sugar levels rapidly, especially in the case of sudden hypoglycaemia. Can include sugary drinks, glucose tablets or sweets (candy).

Vial and syringe

The only way to take insulin for long, many people still use the insulin vial and syringe, consisting of a needle, a barrel and a plunger.

VLDL

Very Low Density Lipoprotein (the “very bad” cholesterol)

Young-onset type 2

Anyone diagnosed with type 2 under the age of 45

Thanks to COVID – 19, the whole world has been under a situation of a near total lockdown. Except for the most essential services like the medical profession or police almost everyone has been confined to their homes for over 2 months. Working From Home (or WHF as it has now popularly known) which was common in the information technology (IT) sector even prior to COVID – 19, became the only way of working for the majority of people irrespective of their profession that one was in. The field of telemedicine was also born in India, thanks to the COVID – 19 and the lockdown!

So, is working from home good or bad, particularly from the health point of view. Undoubtedly, working from home provides many benefits particularly for working mothers. For some others, working from home does not make any difference to their job, compared to physically working in an office. However, working from home can have several undesirable side effects as well.

Not everyone has the luxury of living in an independent house or a gated community where there is scope to exercise either by walking around the house or on a terrace. Many well to do individuals even have a gym or at least a Treadmill at home. However, the average middle class person would be confined in a small flat most of the time. This would almost certainly mean that his or her exercise levels would drastically come down. It is generally recommended that everybody tries to get 10,000 steps a day. If one was able to go for a walk in the morning, one could easily accumulate 5,000 – 6,000 steps. When going to an office for work, some level of commuting in addition to walking about within the office will facilitate completing the 10,000 steps by the end of the day. When one is confined to one’s home, it would be extremely difficult to get 10,000 steps. Over a period of time, this would mean that the calories consumed would not be expended. This can lead to weight gain and obesity and to either precipitation of diabetes or worsening of the Diabetes State, hypertension and even to heart disease.

At home, one is mostly confined to a chair and possibly to a computer. This could mean prolonged sitting which can affect the health in many ways including back problems, disuse of muscles, strain on the neck and the eyes. Indeed sitting as they say, is the new smoking. The diet also tends to be irregular and most people confess that they snack much more when they are confined to their homes. At office, the schedule includes fixed ‘lunch’ and ‘break’ times. Perhaps the greatest toll is on the mental status. Human beings need to interact with others and in an office, one tends to meet many people and thus have an outlet for one’s emotions and also get help with some of their job related problems.

When one is confined to the four walls of one’s home, this often leads to depression, anxiety and stress. To some extent, this can be overcome by taking breaks, relaxation, having a hobby and by yoga, pranayama and meditation. However, one still gets the ‘cooped up’ feeling and this can be quite frustrating in the long run. Thus while, in the initial days, working from home might seem like a boon, in the end, it often turns out to be a bane. Having said that, there are many people who successfully work from home and are able to balance their work life situation very well. It is up to the individual to look after his or her health even if working from home.

Possible solutions :

-Using ergonomically designed chairs.

-Wearing special goggles which protect against the ‘blue light’ of the computer screen.

-Better Mattresses to protect the back.

-Eat more vegetables, fruit and protein.

-Do indoor exercises, yoga, meditation and Pranayama.

-Spend time with family, relax and develop a hobby.

-Take enough breaks and finally

-Sleep well and on time.

By following these simple measures, one can convert working from home into a boon rather than being a bane.

However, Dr. Mohan's Diabetes Treatment & Diabetes Care department said, "there is a protein called adipsin, which is produced in body fat that helps protect insulin-secreting cells called pancreatic beta cells from the destruction of type 2 diabetes.

Among middle-aged adults, higher levels of the protein in the blood were also associated with protection from type 2 Diabetes Mellitus.
