Participant's Release Form for OnlyFans, Fansly and other adult sites
A Participant's Release is an agreement between a model, a photographer and a 3rd party that gives the photographer the right to publish, sell, distribute and do other non-defamatory things with the photos of the model.
When you register on an adult site, you agree to its rules, which are not made up, but written in accordance with the law and the rules of payment systems.
So you have to confirm your age, don't violate copyrights, don't post forbidden content.
And, of course, you must confirm that any other person with you in the frame is of legal age, that you are not holding them by force, and that they do not mind you selling this content and making a lot of money.
That's what you need to sign a Participant's Release Form. Better yet, a million-page disclaimer of all claims, so you don't get fucked over by the DMCA later.
And photographers, videographers, models you shoot content with, and even the owners of the apartment/studio where you shoot can claim copyrights (and send DMCA abuses) because "the design of the apartment is unique and I did not consent to such shooting and for commercial purposes". It sounds ridiculous, but it does happen.
The other person in the content means: the whole person, an arm, a leg, a pixel of a penis, a finger, etc. No matter what part or how much of the other person is visible, you still need to verify it.
Each site has its own process for verifying the second, third or tenth model on the stream/content. For example, somewhere you need to create a new paired account, somewhere you need to upload documents with every video you post, and somewhere you need to send documents to support.

If the model is already registered on OnlyFans, you just need to mark her @username in all posts and messages; if the model doesn't have an OnlyFans account, you need to fill out participant's release form and send it + photo with the printed release and passport to [email protected] support. FAQ.
Fansly
If the model is already registered, you just need to mark her @username in all posts and messages; if the model is not registered, you need to send a signed participant's release form, passport/ID card photo and selfies with passport/ID card to [email protected]. FAQ. Any standard release will do, like JFF, MV or random. Or you could reprint the OnlyFans release, replacing everything there with Fansly.

Example of completing a release for OnlyFans:
The form explanation:
- In the first box, enter the First and Last name of the model on whose page the content is published.
- In the second box, type her @username.
- Then @username one more time.
- Choose the duration of the release (one year, two, five, forever. If you choose one year, then you either have to delete the content or re-sign the release and resubmit).
- The type of document the second model provides (passport or ID card).
- ID number.
- Country.
- Date of birth.
- Name and Surname of the second model.
- Signature of the second model (handwritten Name and Surname in English).
- Date the contract was signed.
How to fill out the Participant's Release Form:
- You can print the form, fill it out in by hand and then scan or take a picture.
- OnlyFans release: PDFs can be opened and filled out directly in your browser, adding a signature in photoshop or Paint.
- PDFs can be filled out and signed online using Sejda (upload PDF, fill in to sign, click Sign and sign the document, when done, click Apply Changes and save the document) or in Adobe Acrobat.
